- Managed care: MCO internal appeal within 60 days of the Notice of Adverse Benefit Determination
- State Fair Hearing within 120 days of the Notice of Appeal Resolution (within 10 days to keep services); FFS/waiver: State Fair Hearing within 60 days of the notice
