- Receive county/state or MCO notice of action
- (Managed care optional) file MCO appeal - not required before State Fair Hearing
- Request State Fair Hearing (DHS-0033) within 30 days (before effective date / within 10 days to keep benefits)
- Fair hearing before human services judge; Commissioner issues order
- Reconsideration within 30 days or appeal to district court within 30 days
